When I wrote my book, YOUR LIFE, Interrupted (published in 2008), it included a form called wallet communications. The instructions were for the user to always carry the card in their wallet, purse, or pocket so that emergency responders or healthcare professionals could access the information quickly. However, there are many other uses for this same form.
For example, if you transport your grandchildren or youngsters, you can fill out this form and attach it to each child’s side of the car seat. Then, if you get into an accident and you cannot speak (and the child cannot speak for themselves), this can ensure the child is taken care of properly.
When you have to go to a new medical facility or service provider, and they ask you to fill out a completed form of everything you have ever had wrong with you or all of the medicines you are currently taking, use the content on this form to complete this repeatedly pain-in-the-neck request.
And, if you travel with your friends and don’t know exactly what they take for medicine or if they have any allergies, have each traveler fill one out and put it in their wallet. Should any of your buddies get hurt on vacation, you can assist the healthcare professionals by accessing and sharing the form.
Consider sharing this information with family members or close contacts so they know your preferences and can assist in an emergency. You never know if/when they might be called about your details when you cannot respond on your own.
